SICKNESS CERTIFICATES
An NHS certificate is not needed if you are off work for less than 7
days (including the weekend); this period is covered by a self-certificate available
from your employer. You need to see a doctor if absence is longer, or your employer
requires a private certificate for which a charge is payable.
NON-NHS SERVCES
Some services are not covered by the NHS and a charge is payable eg:
Pre-employment and HGV medicals
PPP verification
Sick notes for the first week of illness
Prescription for medication to be taken abroad
BUPA verification
Other private certificates
Vaccination certificates
A list of the fees is available at reception.
STAFF
TRAINING
The practice will be closed for staff training on occasional Wednesday/Thursdays each month from 1 - 4pm there will be a poster on the reception door informing you of the date each time and a telephone message also. In the event of an emergency during this time please ring: 01274 581979 and you will be diverted to the out of hours service.
REMOVAL OF PATIENTS FROM THE PRACTICE LIST
We adhere to the NHS zero tolerance policy to protect our staff and other patients. If you are violent or abusive, and this leads to a fear of for a person's safety then you will be removed from the practice list with immediate effect. Please note that the police may be involved.
Repeated misuse of medical services, for example repeated failure to attend appointments without sufficient prior notice may also result in the removal from the list. It is our policy that you receive two warning letters first prior to removal. If you move outside of the practice area, then you will no longer be able to remain on the practice list. We cannot make any exceptions.

FREEDOM OF INFORMATION
PUBLICATION SCHEME
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
This scheme is available from the practice manager.
