HOW DO I....
REGISTER WITH THE PRACTICE?
This requires you to complete a form, which our reception staff will provide.
We only accept patients onto our list if they reside within our practice boundary
(the practice covers BD17 and some outlying areas - please ask our receptionist
to clarify if you are within the boundary area),
and attend for a health check performed by our practice nurse, where you will
be asked to complete a health questionnaire. Once you have become registered
with the practice you can choose to see any doctor. The practice does not offer
personal lists.
