REGISTER
WITH THE PRACTICE ?
This requires you to complete a form and our reception staff will provide this for you. We only accept patients onto our list if they reside in our practice boundary (see map on back of the booklet), and attend for a health check performed by our practice nurse, where you will be asked to complete a health questionnaire. Once you have become registered with the practice you can choose to see any doctor the practice does not offer personal lists.
If you change your name address or telephone number please let our reception team know as soon as possible. If you move outside of our practice boundary it may be necessary for you to find a doctor in your new area.
